Your insurance company might provide rewards based on your physical activity. Please contact your provider or learn more about some of the opportunities below.
Step 1: Register with Unity Health Insurance: Fitness First
You must contact Unity Health directly or call 1-800-362-3310 in order to register for the Fitness First program before signing up with Rec Sports.
Step 2: Register with Rec Sports
Unity requests a workout total report for a 6 month period. Ask an office clerk at any of our facilities to print off a 6 month workout total report. If you cannot make it into our facility to get your printed workout report, you can fill out this form online Access Total Request Form. You will be emailed a pdf document of your workout totals. Once you receive the workout totals report, you can submit to Unity.
Step 3: Work Out to Receive Your Reward
You must work out 60 times within 6 consecutive months in order to receive your insurance reimbursements. The reward comes in two separate increments of $100 after each six month period, for a total of $200 per year.
Step 1: Track Your Workouts
You will need to track your own workouts using GHC’s Exercise for Excellence tracking form. You can also access this form at the Member Services office at any of the Rec Sports facilities.
Step 2: Work Out to Receive your Reward
You must work out 120 times throughout one calendar year (January 1st-December 31st) in order to receive your insurance reimbursement of $100 or maximum of $200 per household.
Step 3: Get Signature and Send in Paperwork
Following completion of workouts, you will need to bring in your completed GHC’s Wellness Reimbursement Form for a Rec Sports employee to sign, along with your completed Exercise for Excellence tracking form to verify your 120 workouts. Mail both forms to GHC. If you cannot make it into our facility to get your printed workout report, you can fill out this form online Access Total Request Form. You will be emailed a pdf document of your workout totals. Once you receive the workout totals report, you can submit to GHC.
You must submit your reimbursement forms to GHC by January 15 of the following calendar year.
Step 1: Purchase a Membership with Rec Sports
You must purchase a membership at any of our Rec Sports facilities and make sure to get a receipt.
Step 2: Send in Paperwork
You must print the Good Health Bonus Reimbursement Request form from the Physicians Plus website. After completing the form, mail it to Physicians Plus along with a copy of your receipt.
Step 3: Receive Your Reward
You are eligible to receive up to $100 for single contracts or up to $200 per family contract of insurance reimbursement within 6-8 weeks of sending in both the Gym Reimbursement Form and a copy of your receipt.
Dean does not require you to track your individual workouts. However, their Living Healthy Rewards program does keep track of points for various activities. This includes an annual health assessment, which gives you a reward of $50. You can receive up to $150 in rewards for one calendar year. Be sure you submit your assessment, complete your activities, and redeem your points by December 31.
