Did you know that if you are a member of Unity Health Insurance, Group Health Cooperative, or Dean Health Plan you could be eligible for a reward or reimbursement for utilizing Rec Sports facilities? Each plan has its own requirements to receive the reward (please scroll down to review each plans's requirements).

UNITY HEALTH INSURANCE

Eligibility:
Students, Fac/Staff, Spouses and Domestic Partners are all eligible to receive insurance reimbursement. 

Step 1:  Register with Unity Health Insurance: Fitness First
All patrons must contact Unity directly at www.unityhealth.com or call 1-800-362-3310 in order to register before signing up with Rec Sports. 

Step 2:  Register with Rec Sports
All patrons will need their Unity member number to register with Rec Sports.  Students and Fac/Staff can ask an office clerk at any of our facilities to sign them into our online tracking system that electronically tallies each time their ID is swiped upon entrance into a Rec Sports facility.  Spouses and Domestic Partners have to manually track their workouts, because their ID’s do not swipe.  They can request a paper workout log from one of our Office Clerks.  Then, every time they workout, they just need to stop by the office to have the Office Clerk initial by the date on their sheet. 

Step 3:  Workout to Receive your Reward
All patrons must workout 60 times within 6 consecutive months in order to receive insurance reimbursements.  The reward comes in two separate increments of $100 after six months, for a total of $200/year.  If both spouses/domestic partners are Rec Sports members, there is a limit of $100 reimbursement per person.

GROUP HEALTH COOPERATIVE

Eligibility:
Fac/Staff, Spouses and Domestic Partners are all eligible to receive insurance reimbursement.  (Students are unable to receive insurance rewards.) 

Step 1:  Register with Rec Sports
All patrons will need their GHC member number to register with Rec Sports. 
Fac/Staff can ask an office clerk at any of our facilities to sign them into our online tracking system that electronically tallies each time their ID is swiped upon entrance into a Rec Sports facility.  Spouses and Domestic Partners have to manually track their workouts, because their ID’s do not swipe.  They can request a paper workout log from one of our Office Clerks.  Then, every time they workout, they just need to stop by the office to initial by the date on their sheet.

Step 2:  Workout to Receive your Reward
All patrons will also need to bring in a Wellness Reimbursement form for a Rec Sports employee to sign, verifying their 120 workouts.  Patrons can print this form from GHC’s web site, http://www.ghc-hmo.com/exercise_for_excellence.asp

All patrons must workout 120 times throughout one calendar year in order to receive insurance reimbursements.  The reward of $100 will be sent after GHC receives and processes the Reimbursement form.

DEAN HEALTH PLAN

Eligibility:
Fac/Staff, Spouses and Domestic Partners are all eligible to receive insurance reimbursement.  (Students are unable to receive insurance rewards.) 

Step 1:  Purchase a membership with Rec Sports
All patrons need to purchase a membership at any of our Rec Sports facilities and ask for a copy of their completed membership form as their receipt.

Step 2:  Send in Copy of Membership Form and WIN Reimbursement Form
All patrons must print off the WIN Reimbursement Form from Dean’s web site (http://www.deancare.com/healthplan/shared/ComplementaryHealth/win.asp?V=1), and select “Fitness Focus” as the WIN Program Eligible Service.  After completing this form, they need to mail it in to Dean Health Plan with the copy of their completed Rec Sports Membership form (that they got at the time of purchase) before 90 days after the end of their contract year.

Step 3:  Receive your Reward
Patrons will receive up to $100 of insurance reimbursement within 30 days after sending in their Membership Form and WIN Reimbursement Form.
